To apply for a new USA Passport, you will need to appear in person at a post office or courthouse that accepts passport applications. Use the form below to find the US passport acceptance office nearest you. A. Proof of CitizenshipTo prove your citizenship, you must have an original or certified copy of your birth certificate, naturalization papers or old passport. B. Proof of Identity In order to prove your identity to the acceptance agent, you will be required to show a driver's license or state-issued identification issued more than six months ago. Official government or military picture identification, are also acceptable. C. DS-11 Application for a New Passport Click on the link at the bottom of this section ("Section C") to create your DS-11. You will be asked to input your personal information. After inputting your personal data, click on "Create Form" which will create your completed DS-11. D. DS-64 ApplicationComplete the DS-64 Statement Regarding Lost or Stolen Passport Form. On the questions, “How was passport lost or stolen” and “What efforts have you made to recover the passport,” you should answer both questions in detail. Click here for the DS-64 Form E. Passport Photos Two (2) identical passport photographs. Go to a passport photographer to have the photos taken. F. Proof of Departure Submit a copy of your computer-generated itinerary or your ticket from an airline, travel agency, or online booking agency showing your departure from the US FAX or photocopies are acceptable. Please include one copy inside the sealed envelope and one copy outside of the sealed envelope. G. Government Fees Two checks or money orders, one payable to US Department of State for $127 and the second for $30 payable to the courthouse or post office (the acceptance agent fee). H. Letter of Authorization A letter of Authorization giving us the authority to submit your application and pick-up your passport when issued. Provide two originally-signed authorization letters - One copy should be inside the sealed envelope and one copy outside the sealed envelope.

While completing the online order form in this step, you will be asked how you want to pay AAA Passports. You may pay our fees online with a credit card or enclose a personal or business check or money order. Processing times are business days and do not include shipping time. On the FedEx air bill, check the box for "Priority Overnight". On the DHL (Airborne Express) way bill, mark the box for "10:30 Delivery". For shipments via USPS Express Mail, Air mail, please follow the instructions carefully when completing the air bill. Failure to follow these instructions may cause a delay in processing your application prior to your departure. |
